Mental Health Case Management Services

Case Management services are available to adults with serious and persistent mental illness and to children and adolescents with emotional and behavioral disorders.

Case managers help clients and their families to access therapeutic, medical, dental, educational, recreational, vocational, residential, financial and other resources. The goal is to assist each client in living successfully in the community to the greatest extent possible.

Interac offers two levels of case management services :

* Intensive Case Management (ICM) is available to adults who are at risk for frequent psychiatric hospitalization and use of emergency services. Case loads are small and each client is engaged in a supportive relationship with his/her case manager. Specific activities are flexible and are guided by each client's individualized service plan. ICM services are available 7 days per week, 24 hours per day. ICM also provides specialized services to mental health clients who are deaf or hearing impaired.

* Resource Coordination (RC) is available to adults, children and adolescents. RC is goal oriented and geared toward solving specific problems such as need for housing, medical care or entitlements.

For additional information, contact Interac's Centralized Intake Unit at (215) 487-1330 or TTY (215) 487-9529.
