| Community Hospital Integration Program |
The Community Hospital Integration Program (CHIP) is an innovative individualized leisure and recreation program for older adults with serious and persistent mental illness and a history of long term psychiatric hospitalization.
Working with the client, the residential provider and the case management team, CHIP staff develop an individual plan of recreation and leisure activities based on each client's interests. Activities take place during the day, in the evenings and on the weekends, stressing community participation and individual choice. Some activities include :

Arts/Crafts
Church/Temple
Community Senior Centers
Concerts/Movies/Theater
Exhibits/Galleries/Museums
